Social Worker / Discharge Planner

The Social Worker / Discharge Planner is responsible for facilitating psychosocial assessments; initiate contacts with a variety of community resources including the court system; maintain contact with family members, per need and/or patient request; provides clinical supervision to the clinical team; conduct groups; conduct chart audits for regulatory compliance and patient safety; assists with departmental policy and procedure development; provide social service employee evaluation input; and implement discharge planning. 



Job Responsibilities/Duties

  • Expertise in the following areas will be Psychosocial assessment, Legal aspects regarding psychiatric care, Discharge planning, Community resources and Group facilitation.
  • Evidence of skills in communications (both written and oral), organization and time management.    
  • Ability to relate to and maintain a rapport with co-workers, patients/clients, families and representatives of community resources.​


Qualifications

Minimum Education: M.S.W., BA or BS from an accredited school of social work required. Marriage & Family Therapist may be considered.

Minimum Experience: Minimum of five (5) years experience in a related Social Services field, preferably healthcare.

Req. Certification/Licensure: Current California Behavioral Science as an LCSW. Current American Heart Association (BLS) certification. Current CPI or MAB certification.
