Case Manager

The Case Manager performs social services job duties that include case management, initial  psycho-social assessments, treatment planning and discharge planning and after care coordination for psychiatric patients in an acute inpatient psychiatric hospital. 



Job Responsibilities/Duties

  • Attends all treatment team meetings and coordinates discharge plan with all members of the Treatment team.
  • The Case Manager will carry an active caseload of approximately 10-18 Psychiatric patients and/or perform group therapy under the direction of licensed personnel.
  • All other job duties as assigned by Director of Case Management and outpatient program.
  • Facilitates and coordinates all non high risk discharges and planning issues.
  • Identifies age-related problems that may require intervention, e.g. geriatric patient for visual, comprehension, gait or housekeeping problems and the ability to participate in the discharge plan.  
  • Utilizes an interdisciplinary approach to develop the plan and goals, as appropriate.
  • Facilitates and coordinates all non high risk discharges and planning issues. Identifies age-related problems that may require intervention, e.g. geriatric patient for visual, comprehension, gait or housekeeping problems and the ability to participate in the discharge plan.
  • Assists Social Services in planning setting that is most appropriate to meet the patient’s needs.


Qualifications

Minimum Education: BA required with work related experience. Master’s degree in Social Services or related field preferred.

Minimum Experience: Minimum of three (3) years in a hospital setting. Bilingual in Spanish / English is preferred.
