Social Worker / Discharge Planner

A Social Worker/Discharge Planner will facilitate psychosocial assessments; initiate contacts with a variety of community resources including the court system; maintain contact with family members, per need and/or patient request; conduct groups; and implement discharge planning. The Social Worker will be under the supervision of an LCSW/LMFT. 

Job Responsibilities/Duties

  • Demonstrates competency in job-specific skills, including but not limited to age specific needs; group facilitation; community resources; interdisciplinary treatment planning. 
  • Facilitates patient’s completion of Release of Information form.
  • Incorporates treatment plan with other members of the health care team, in and outside of hospital, utilizing appropriate methods (form, phones, etc.)
  • Consistently modifies social service plan and discharge status. Documents all relevant information, including family / community names and phone numbers, on the Aftercare Instructions form in patient’s chart.
  • Works with the clinical team and outside personnel (i.e., public guardian) to plan the setting that is most appropriate to meet the patient’s needs.
  • Actively participates in clinical rounds and interdisciplinary treatment planning 
  • Contacts attending physician, medical director or BHU Administrator as needed, for any unusual or high risk conditions re: patient’s discharge status.
  • Identifies all special needs that may require intervention.
  • Coordinates patient transfer to other hospitals.
  • Performs other related duties as assigned.


Minimum Education: Bachelor’s degree in psych related area of study. MSW or Masters’ Degree in psych-related area of study preferred

Minimum Experience: One (1) year of experience in an acute hospital setting preferred. Experience in the following areas: Psychosocial assessment, Legal aspects re: psychiatric care, Discharge planning, Community resources, Group facilitation. Evidence of skills in communications (both written and oral), organization and time management. Ability to relate to and maintain a rapport with co-workers, patients/clients, families and representatives of community resources.

Req. Certification/Licensure: Current BLS certification; MAB / PART/ Pro-ACT / CPI / ARC / Violence Prevention and De-escalation Healthcare certification.
