The Admission Nurse welcomes and greets incoming patients transferred from other facilities. He/she will assess and evaluate incoming patients to determine the severity of illnesses and assists in appropriate and expedient patient placement within the hospital. This nurse will be a patient advocate and ensure the effective transition of care and delivery of services.
Receive notification of pending patient admission.
Facilitate appropriate bed and room assignment.
Ensure expedient registration of admitted patients to the hospital.
Accompany patient to designated and appropriate room within the hospital.
Provide information about what to expect during hospital stay and provide welcome bag.
Complete required admission assessment, inclusive of medication reconciliation.
Notify attending physician of patient and acquire physician orders.
Track physician non-response to calls for orders and other necessary items for patient care.
Provide appropriate hand-off communication to assigned nurse (no registry).
Assist with discharging patients, if time permits.
Round on all transferred patients throughout hospital stay (check-ins).
Minimum Education: Graduate of an accredited nursing program
Minimum Experience: Minimum of one-year nursing experience required (no registry)
Req. Certification/Licensure: Active, current, and unencumbered Registered Nurse licensure by the state of California.
